1) elderly, but not too elderly. I don't want to reach the point where I'm a crumpled old man that can't even walk.
2) quick way, 'cause it'd be quick...
3) atheist. If by "state your beliefs" you're referring to what I believe about death: you can destroy someone's personality and memories by destroying parts of their brain, so there is no reason to assume that there is any kind of 'soul' surviving in an afterlife when the whole thing is destroyed (aka death). So death is just like before you were born: non existence and nothingness.
