this is an easy question basically type in an incorrect code 4 times into the lock code this will block your phone creating a PUK code
which will then mean you will have to phone your service provider ask them for the PUK code this will unblock the first set of codes then you can input a new code
and so your phone will now have a new code and working just like before.
