OK, Jane Seymore had a tv show where she was some kind of western doctor. They had a co-star, male, very muscular. He killed himself - for real! - on day on the set by placing a revolver to his head and pulling the trigger. The blank pushed his skull into his brain and killed him.

Dumb, dumb. Happened about 15 years ago.
