Ask him if he ever wants a family, or if he wants to live to see his 15 year class reunion.
And tell him too that if he lost just 50 pounds, and cut down to one pack a day, it's amazing how much better he'd feel. People that are self- abusive like this really don't realize this until they start changing their habits for the better, but he will feel better, and probably won't die in ten years, if he'd do what I suggest.
