What is your mobile model? if your mobile is nokia then try with default code "0000" or "1234" and if it does not work then try with unlocking4free to get master reset code for your mobile by just submitting the imei of your mobile and get master reset code to reset your mobile...

Whats your Mobile model ? If the default codes are not working, use Master reset code . you can get it from the provider Unlock-free.com to reset it .
