record incident report?

I hope someone can help me. I want to legal record what is called franchised companies incident report. I no longer trust the company, the franchise or the union or union steward. I will only trust me or
a lawyer. I'm in Ontario Canada. If someone can give me the written document or where to find written
law. I need it in writing so i can hand it to the franchise or manger and a not working or caring union
