Nissan Almera/Sentra QG18DE loss of power in open loop?

When the car is cold started the idle is erratic for a few seconds, then returns to a normal idle after 45 seconds or so. When the car is started warm, the idle is perfectly normal and stays constant.

Under heavy acceleration, the car seems to be sluggish, and refuses to go over around 5,000 rpm, and there is the feeling of knocking or misfiring in the engine when the rev range gets this high in open loop (the rev limiter kicks in at about 6,500rpm).

The engine sometimes shows the fault of P0171 (system too lean bank 1) and P0300/301 (misfire cyl. 1), and from the freezeframe data, the errors always described by the ECM as 'OPEN LOOP- FAULT'.

Ive checked the MAFS, HO2S 1 & 2 and air temp. sensor, all are fine (but to my knowledge these aren't used in open loop anyway). I was thinking it is most likely to be the injectors or fuel filter etc, but i'm no expert! Is there anything else worth checking before going to the trouble of removing the injectors?
