If i get a smartphone for example the Lg Incite for At&t do i have to add any...

Maybe, some smartphones will only work if you have internet on them ($40 more a month) but some don't. You will have to ask the provider what phones require what.

No, you do not have too although if you do the phone may be cheaper to buy from AT&T.
