who sets fashion trends? and how many people really follow?

When they talk about an article being in fashion, like the colour of clothing, or jewlery. Basically, when you find a really nice pair of jeans thats very very cheap cause its 'out of fashion', how many people seriously care?

I guess designers and people who have to have the latest thing set fashions. I am not and have never been a trend setter of fashion followers.
