They are going to ask you your zip code. They have no idea and couldn't care less what city you are calling from. That has nothing to do with the number you will receive, unless you ask for it specifically.

yes.They need your home address and mailing address and place of work and contact information.
so yes they will need your area code and they will give you a phone number based on where you will be using your phone.
