Write down all the problems and then take or send it to the Human Resouces Department.
There's probably a corporate phone # somewhere around the office. I'd check online for a website that you can use to contact "higher ups" (people who are in charge of the company in a larger, more powerful way)

if you've got a union then complain to your union rep. they have influence with the higher levels of management otherwise you'll have to take it into your own hands to bring it higher up in the chain of command. and if he/she is the highest level...then it's time for you to either suck it up or quit
