Who do I choose?

I logg on today and click iTunes.
It takes me to this set up AGAIN! It always does this. I always click ok and then my iTunes goes back to normal and I just loose my downloaded stuff. I come on today and instead I accidentally click "CANCEL" and it goes straight to iTunes and nothing is there. It all got deleted.
What do I do?
How can I make that set up thing go away?
