My girlfriend in a emergency slammed on the brakes, hard stop from 50 mph. The force seemed to pull apart the coolant hose leading to the engine. I put it back together and tightened the clamp back up, and after 10 minutes of driving pressure seemed to push the hose off the clamp and coolant went everywhere.

My question is, did this happen because I didn't correctly put the clamp on, or is there some issue causing pressure build up?( I'm confident I put it on correctly, however)

Also keep in mind the vehicle never had an issue prior to the hard stop and hose disconnecting. Also I'm aware there is a lot of hoses in the car, best description is it was on the lower right of the engine block and disconnects in the middle of the hose, doesn't directly connect to the engine or surrounding parts.
