how to reinstall handheld showerhead that fell off?

A friend of mine was using my shower, and he was too rough with the handheld shower thing, and it fell off. I cannot figure out how to make it stay on. It snaps onto a ball-shaped end, and there's a screw-tightened metal cuff that supposedly holds it on. But when I tighten it all the way, it just falls off again when I turn on the water. There's an articulated rubber sleeve on the shower pipe. I can't figure out if it's supposed to fit under something to make the fit tighter. In any case, I can't make the sleeve fit under anything, so it's sitting there loose. There's a little washer in the assembly that I didn't lose. As far as I can tell, there are no missing parts, but the showerhead won't stay on the pipe.
