What Is The Copyright Form Needed For Selling An Educational DVD Through Stockists.?

My teaching company has made a D.V.D. that we sell to students and non students.
We have interest with other businesses stocking and selling the D.V.D. and one has asked for the copyright form?
Can I downoad one or not?
We do have the copyright notice on the D.V.D. insert cover.
Any advise wouldl be so gratefully received. Thanking you in advance.
