I love using allrecipes.com for finding most recipes. It is a good site. For the recipe from a long time ago my guess is Betty Crocker as they are one of the oldest cookbook and everyone used them awhile ago.

For the broccoli topping, try a sleeve of ritz crackers mixed with a stick of butter and two cups of grated cheddar cheese. Crush the crackers...add the softened butter and the cheese. Then sprinkle on top. I usaually put some of it in casserole, then put the rest on top.
